There are various forms we require of all "campers" and they are determined by your age group.
These files are saved as Portable Document Format (PDF) files. You
will need a PDF reader to access them. If you do not already have

Please note that the forms required by STORM Camp are the same standardized forms used by Northern Pines Camp. Please fill them out completely even if items do not seem to be applicable to STORM Camp. Please determine your age and choose the appropriate form below.

All Participants
This is the form we need to know your initial interest in attending STORM Camp. We have a limited number of openings so sign up know to hold your place in this years awesome event. The 2010 camper fees are:
$275 if registered by 3-31 ($75 Non-refundable Deposit) $300 if registered after 3/31/2010
We expect all fees paid by June 1, 2010
Adult/Driver fee is: $265

These forms must be filled in entirely BEFORE any youth can participate in any camp activities. These forms are a legal requirement and help us provide a safe, fun and healthy time for any of our youth participants. Select and print the forms out.

Background Check Form (PDF)
23 and older
Adult volunteers, drivers, organizers, group leaders or those doing anything else at Camp also need to fill out the two forms above so we have all pertinent information on file as well. Adult drivers must allow us to conduct a background check in order to participate as a driver at STORM Camp.
